A plea from your friendly neighborhood SCUBA diver.....

Please. Please!. PLEASE!!!! In the name of all that is good a holy and by the grace of BOB herself, check your medical forms before you send them back to camp!!!! I have never seen this many screwed up forms, EVER. Take five minutes, look it over, make sure it was a doctor that signed your physical, make sure all the signature and inital blocks are signed, read everything over, and then send it. I've got a list longer than my arm of people I need to contact tomorrow because their forms are screwed up and if people would just take a minute and look things over, this wouldn't be an issue.

This is the first year I didn't look over the forms before my dad sent them. Here's to hoping that my trust wasn't too misplaced.

The forms are pretty self-explanatory, aren't they? Just read along and follow the directions...

No fret, I'll make your life less chaotic and fill in my forms appropriately. If you're lucky I might even include a copy of my Open Water SCUBA certification.
