Definitely your call sign! Although, depending on which counsellors you have, they could give you a new call sign at Aviation Challenge. So, I'd wait to get your name tag printed until after your first day there. Same goes for dog tags... if you want them, wait until you are at the camp for a day or two before you have them made.

make sure you proofread your form before you hage your tags made up. i was notorious for making mistakes on my dogtags and not noticing it until i was already at home:-P

i think they mean name or prefered name. this is for the name tag you have on all week with your name and hometown (as well as mail they have to send you between now and when you leave for camp) for instance if your name is james smith but you prefer jim and you live in jackson mississippi, your name tag would read:
jim smith
jackson, ms

please put the name you go by with your family, friends and school people. DON'T put a nickname like "Hawk" or "Shorty" especially if you're going to AC, because we'll give you a new callsign anyway, and we need to know what name to announce for graduation as your first name.

I'm a bit confused by your usage of the word "sick."

But yes, at Aviation Challenge (or Aviation track Academy), you do get call signs. However, your team chooses your call sign, NOT you. And there's a list of call signs you can't get (any counselor from the previous two years and any character in Top Gun).

I've heard sick used like that - but I've also heard sick to mean it's intended meaning. So I wasn't quite sure which meaning he meant - and since I couldn't hear how he said (I presume Rogue Leader is a he; correct me if I'm wrong), I asked for clarification.

I've heard all kinds of crazy slang, but most of it makes way more sense HEARING it than reading it.

Yeah the nurses will definitly have a problem with tags that don't correspond. I suppose that its quite understandable though.

And make sure your medications are in English... My pharmacy was bright enough to put the 'take with food' label on the box over the English dosage chart. The nurses weren't pleased, but they believed me when I deciphered the French chart for them. I don't think this is a problem in the US, the double language thing, but I thought I'd mention it anyway just in case they have English-Spanish.

One guy on my team put down his callsign from his last time at AC for his nickname, so his name tag said "bubbles". It was sad.
